Online Donation form - Amount options

Online Donation Forms can now have Amount Options.

You can have either Radio Buttons or they can be in a drop-down list.
Each amount option can optionally have a Description and Memo. For example: $180 Chai Supporter, $360 Builder. Memos can be used to describe each option in more detail.
